Which GBP Category Should a Home Care Agency Use? The definitive answer

Your Google Business Profile category is the single largest relevance signal Google uses to decide whether your agency matches a local search. Getting it wrong costs you Map Pack positions that no amount of reviews, citations, or backlinks can recover.

Why category selection matters more than most agencies realize

Google's Map Pack algorithm weighs three factors when deciding which agencies appear for a local search: relevance, distance, and prominence. Relevance is how well your listing matches what the searcher is looking for. Distance is geographic proximity. Prominence is how well-known and trusted your agency is across the web.

Your GBP primary category is the biggest lever you have over the relevance factor. It tells Google what your business fundamentally is, before Google reads your website, before it processes your reviews, and before it evaluates your citations. A wrong primary category suppresses relevance even when every other signal is strong.

Across our client data, category mismatches are the most underdiagnosed reason agencies underperform in Map Pack rankings. A typical scenario: an agency has 80 reviews, a complete profile, and clean citations, but ranks at position 6 because they selected a category that does not match the dominant search intent in their market. Fixing the category alone has moved agencies from position 6 to position 2 within six weeks.

#1 Ranking factor category affects

Relevance, the first of Google's three core Map Pack signals, is controlled primarily by your GBP category. No other single field in your profile has more impact on which searches your listing appears for.

The answer: your primary category

For the large majority of home care agencies, the correct primary category is Home Health Care Service. Here is why that matters.

Home Health Care Service
Recommended Primary
Highest search volume of all home care-related GBP categories. Matches the dominant query intent for families searching for in-home assistance, personal care, and companion services. Google surfaces this category for searches including "home care agency near me," "in-home caregivers," "senior care at home," and related terms.
Home Care Service
Acceptable Alternative
Lower average search volume than "Home Health Care Service" in most U.S. markets. Can be appropriate if your agency is primarily residential housekeeping or domestic services rather than personal care and companionship. For agencies providing hands-on personal care, "Home Health Care Service" consistently outperforms this option.

A common question is whether the word "health" in "Home Health Care Service" implies medical or skilled nursing care. The answer from Google's category taxonomy perspective is no. The category covers the full spectrum of home care, including non-medical companion and personal care services. Google's own category descriptions and the agencies that rank well for non-medical care searches confirm this.

If your agency provides skilled nursing, physical therapy, or other medically licensed services at home in addition to companion care, "Home Health Care Service" is even more clearly the right primary category.

Category Relative Search Volume Best For
Home Health Care Service Highest Non-medical personal care, companion care, skilled care
Home Care Service Moderate Domestic/housekeeping focus, or as a secondary category
Aged Care Lower (growing) Agencies specifically targeting 65+ with age-related framing

Secondary categories to add

Secondary categories expand the range of searches your listing can appear for. Google allows up to 10 total categories. Add every secondary category that accurately describes a service your agency provides. Do not add categories for services you do not offer; that creates a relevance mismatch that hurts more than it helps.

Senior Citizens ServicesBroadens your listing's relevance to senior-focused queries that don't include "home care" specifically. Highly recommended for all agencies.
Elderly CareA companion category to Senior Citizens Services. Adds coverage for "elderly care" query variants without overlap problems.
CaregiverCaptures searches from families looking for individual caregivers. Useful even for agencies that place caregivers rather than employing them directly.
Aged CareA newer category in Google's taxonomy. Growing search volume. Add it now to get ahead of agencies that haven't updated their profiles.
In-Home Nursing CareAdd this if your agency provides or coordinates any skilled nursing services at home. Relevant for both private pay and Medicare-certified agencies.
Nursing AgencyRelevant if you recruit and place nurses or CNAs. Even if you employ your caregivers directly, this category can add coverage for healthcare worker placement searches.
Assisted Living FacilityOnly add this if your agency also operates or is affiliated with an assisted living facility. Do not add it solely to capture those searches.
Disability Services and Support OrganizationRelevant if your agency serves adults with disabilities, developmental conditions, or serves clients under Medicaid waiver programs for people with disabilities.

Categories to avoid

These categories are either too broad, carry the wrong search intent, or signal something your agency is not. Each one actively hurts your relevance score.

Health Care
Avoid
This category is so broad it produces almost no Map Pack traffic. Nobody searches "health care near me" when they need home care for a parent. You lose specificity without gaining any search volume.
Medical Service
Avoid
This pulls medical intent, not home care intent. Families searching for in-home personal care are not looking for "medical services," and Google knows it. Mismatched intent means fewer impressions even if you rank.
Social Services Organization
Avoid
This category signals a nonprofit social services entity, not a private-pay or insurance-based home care agency. It attracts the wrong intent and reduces your relevance for commercial home care queries.
Hospital
Avoid — obvious but it happens
Agencies occasionally select this during initial setup when searching for "health care" and landing on the wrong result in the dropdown. Always verify your selected category by reading it back before saving.

How to change your GBP category

The process takes less than two minutes but the steps vary slightly depending on whether you manage your profile through Google Search or the Google Business Profile dashboard.

1
Search your business name in Google
On desktop, search your business name while logged into the Google account that owns your GBP. Your business profile editing panel will appear on the right side of the results page.
2
Click "Edit profile"
This opens your profile editing interface. Navigate to the "Business information" tab if it does not open there automatically.
3
Find the "Category" field and click the pencil icon
Your primary category appears first. Click the pencil to edit. The field will become a searchable text input.
4
Type "Home Health Care Service" and select it
The category must match exactly from Google's taxonomy. Type the category name and select it from the dropdown. Do not type a custom name.
5
Add secondary categories in the same panel
After setting the primary category, use the "Add another category" option to add secondary categories from the list above. Save when complete.
6
Click "Save" and verify the change is reflected
Google typically applies category changes within a few minutes. Search your business name and confirm the category displays correctly in the Knowledge Panel.

Does changing your category cause a ranking drop?

Yes, temporarily. When you change your primary category, Google re-evaluates your listing's relevance for all affected query types. This re-evaluation period typically lasts 2 to 4 weeks, during which your rankings may fluctuate or temporarily decline.

This should not stop you from making the change. If your current category is wrong, the temporary instability is worth the permanent improvement. Across our client accounts, category corrections have consistently produced better final rankings than the pre-change position, with the new equilibrium reached within a month.

Timing tip

Avoid changing your primary category during your agency's busiest inquiry periods (typically fall and early winter as families plan for the holidays). If you must change it during a busy period, plan for 2 to 4 weeks of potential fluctuation and have your other marketing channels ready to compensate for any temporary drop in organic calls.

How to see what categories competitors use

Find a competitor ranking above you in the Map Pack. Open their GBP listing and then view the page source in your browser (Ctrl+U on Windows, Command+Option+U on Mac). Search the source for primaryCategory to find their selected primary category. Their secondary categories appear nearby in the same JSON block.

The PlePer GBP Category Tool (available at pleper.com) provides this same information without requiring you to read page source. Enter a Google Maps URL and it extracts all categories from the listing automatically. This is faster when you are auditing multiple competitors.

Look at the top 3 agencies in your Map Pack. If all three use "Home Health Care Service" as their primary category and you are using "Home Care Service," you have found one contributing reason you are not in that top 3.

Common mistakes

Keyword stuffing in the category field. Some agencies try to type "Home Care Services Phoenix AZ" into the category field. This is not how the field works. It uses a fixed taxonomy; you select from Google's list. Any free-text entry that does not match a valid category name will not save. If you see an agency with location names in their category display, it was either a temporary Google glitch or they have keyword-stuffed their business name field instead, which violates policy.

Using only one category. Agencies that fill only the primary category and leave secondary categories empty leave significant search coverage on the table. If you legitimately provide companion care, personal care, and serve a senior population, you have three or four accurate secondary categories to add. Each one expands the set of queries your listing is eligible to rank for.

Updating categories without updating the business description. Your GBP business description should reinforce your primary category's relevance. If you've just changed your primary category to "Home Health Care Service," make sure your description uses natural language about the specific services you provide: bathing assistance, medication reminders, companionship, mobility support. Google reads this text as a relevance signal alongside the category.

FAQ

What is the best primary GBP category for a non-medical home care agency?
Home Health Care Service is the best-performing primary category for most non-medical home care agencies, even if you do not provide skilled nursing. It has higher search volume than Home Care Service and broader ranking potential across relevant queries.
How many secondary categories should I add to my GBP?
Add every secondary category that legitimately describes a service you provide. Google allows up to 10 total categories. Padding with irrelevant categories can dilute relevance, but 4 to 7 accurate secondary categories is typical for home care agencies.
Will changing my GBP category cause a temporary ranking drop?
Yes. Expect 2 to 4 weeks of instability after changing your primary category while Google re-evaluates your listing's relevance. Rankings typically restabilize at a higher position than before the change if you've switched to a more accurate category.
Can I see what GBP categories my competitors are using?
Yes. Search your target keyword in Google Maps, open a competitor's listing, and view the page source. Search for "primaryCategory" in the source to find their selected category. The PlePer GBP Category Tool also extracts this data without needing to read source code.
Does adding the city name to my GBP category help rankings?
No. GBP categories are a fixed taxonomy maintained by Google. You select from their list; you cannot customize or add text to category names. Attempting to add keyword text violates Google's guidelines and can result in suspension.
